8/14/2023 EnchantaVaughn Approved 1.Behavior: When QMHP arrived, Laylah was in a laid-back mood, as evidenced by her calm and relaxed expressions. Laylah explained that she wanted to have session without mom present so that she could spend that time taking care of important duties around the house (i.e. cooking and cleaning). Laylah is still adjusting to the changes in the home and giving her mom space for self-care and to attend to her brother who also has mental and emotional issues. 2.Behavior: After having one on one with her counselor, Laylah was able to figure out some things that she has been having trouble with. Laylah expressed that she didnât want to be judged for her mistakes. Laylah explained her frustrations and why she has been more stressed than normal lately. Laylah was respectful of the session time and utilized this time to lay everything out on the table. Laylah shared positive and negative thoughts concerning her expectations and responsibilities in the home. 1.Intervention: QMHP conducted session today on decreasing disrespect for authority figures and taking ownership for her own actions. QMHP probed Laylah about her behaviors yesterday. QMHP encouraged Laylah to remain positive and more positive things will happen. QMHP prompted Laylah to refrain from impulsive, hyperactive, defiant, and aggressive behaviors especially running, refusing authority instruction, and interrupting her mother while sheâs working, helping her little brother, resting, cooking, or cleaning. 2.Intervention: QMHP utilized behavior modification strategies including removing Laylah from the stimulant environment in efforts for her to reduce inappropriate behaviors. QMHP encouraged Laylah not to complain and be more grateful. QMHP reviewed Laylah schedule with her and supported as she prepped for the evening work she had. QMHP intervened to assist in explaining behaviors to Laylah and what she needs to work on if she wants to meet her goals and objectives daily. 1.Response: Laylah was highly oppositional during session and displayed argumentative and frustrating behavior. Laylah displays moderate participation unless sheâs distracted by her little brother. Laylah recognized when sheâs not paying attention and quickly corrected herself. Laylah was able to identify both positive and negative behaviors while trying her best to display more positive behaviors. After applying different strategies and restraint by counselor, Laylah was able to stabilize her negative behaviors and contribute to discussion. 2.Response: Laylah has a history of displaying aggressive, non-compliant behaviors towards authoritative figures in the home and/or community setting. This has been the first occasion in which Laylah has displayed this level of behaviors in the presence of counselor. Laylah returned to activity ready to work and was more motivated to try and stay positive for the remainder of the day. Laylah acknowledges expectations and reviews goals and objectives with counselor and parent.
